Creating a case in 4Next standalone

Depending on your authorizations for 4Next, you can create a discharge case for patients who are in-house at your Mass General Brigham site, or for "OUT" patients who are not.

  • If you haven't already:
  1. Click Cases Sent on the main menu.
  2. Click add case.

Mass General Brigham site  - add an inpatient case

  1. Search for the site where the patient is admitted by typing its name (not its acronym) in the Referral Source Provider Name field. Type an asterisk (*) —for example, Mass*— if you don't want to type the complete name. Results include only the sites you're authorized for.
  2. As you type, sites matching your entry are displayed automatically. Click to select the site from the results.
  3. Click next. (If you chose the wrong referral source, click Edit icon Search for a Referral Source and search again.)
  1. Type the patient's MRN in the top field; if you don't have an MRN (at least the first letter of the patient's last name is required):
  • Type the patient's first and last names
  • Press the TAB key to move to the next field without clicking
  • Type (M, F, N, or U) or click to select an option from the Gender drop-down list
  • Type (mm/dd/yyyy) for the date of birth or click  to select from the calendar
  1. Click search for patient or press ENTER.
  2. If there are more than 40 patient records that match your search criteria, a message pops up: "Only first 40 matches are returned. Narrow your search criteria to see additional results." Click ok to close the message. Repeat the steps above with additional information in your search criteria.
  3. If the case is already created for the selected referral source:
  • The message "A 4Next Case is already opened for this patient" displays beneath the search button.
  • The list displays 5 items by default. If there are more than 5 results:
  • To change the number of items per page, click to select from the Items per page drop-down list (bottom right). 

  • To display the next page, click Right-arrow button. To display the previous page, click Left-arrow button. (Scroll up a bit if the page navigation buttons are covered by another button.)

  • For each case found, a line item displays the patient's firstname, middle initial, and last name, PMRN, DOB, sex, acute facility, assigned case owner, and status of the case.
  • Click the link for the name to go to the case in 4Next.
  1. If the patient you want is not listed as an active case, click more patients to display any patients who have Mass General Brigham PMRNs, whose records match the information you entered.
  • To display demographic and visit information for a patient, click the More Details link on the right. Patient details display  in a new browser window.
  • Click the 4Next tab to return to the patient lookup results in 4Next.
  1. To display visits for a patient, click to check the box on the left checkbox checked (yellow), and then click select visit.
  2. To add a case for a visit, check the visit number and admit date carefully, click to check the box on the left checkbox checked (yellow) , and then click create case.
  3. Click createicase. The Case detail for the patient opens.

Add an outside patient OUT case manually

Liaisons are authorized to create a discharge case for encounters that are not at a Mass General Brigham site. If the patient doesn't have a medical record with Mass General Brigham, the liaison can manually add the record to 4Next.

  1. Search for the site where the patient is admitted by typing its name (not its acronym) in the Referral Source Provider Name field. Type an asterisk (*) —for example, Mass*— if you don't want to type the complete name. Results include only the sites you're authorized for.
  2. As you type, sites matching your entry are displayed automatically. Click to select the site from the results.
  3. Click next. (If you chose the wrong referral source, click Edit icon Search for a Referral Source and search again.)
  1. Type all of the following information (press TAB to move to the next field):
  • Type the patient's first and last names
  • Press the TAB key to move to the next field without clicking
  • Type (M, F, N, or U) or click to select an option from the Gender drop-down list
  • Type (mm/dd/yyyy) for the date of birth or click  to select from the calendar
  1. Click search for patient or press ENTER.

  1. If the patient has a Mass General Brigham record, click the box on the left to select checkbox checked (yellow) the encounter as a new case. (If not, continue to create a record for the encounter.)
  2. Click createicase.
  3. If a Mass General Brigham record was available and you did not select one, when prompted to confirm that you want to create a new case, click no to return to the lookup, or yes to create the new case.
  4. If you're creating a new record, review the information you entered and update any that is incorrect, and then click createicase again.
