Add service and medical equipment needs to an existing case

To begin a referral for a selected patient, you must select the patient's needs to search for providers of services and/or medical equipment. Once it's complete, you can filter the results of your search to narrow it down to providers that are the best matches for a referral.

  • If you haven't already, from 4Next standalone:
  1. Click Cases Sent on the main menu.
  1. Click the patient name to select a case.
  2. Click Manage.
  3. Click add needs in the bottom right to open the Add Patient Need view.
  • If you haven't already, from DHeCare (Epic Hyperspace):
  1. Log in to the Case Management or Care Coordinator module.
  2. From the patient encounter, click the 4Next tab.

green clipboard Indicates that a provider search was run at least once already for the need. If you search more than once for a need, providers who are already added to the case can't be selected. (See Filtering the referrals to show providers with an Inactive status.)

  1. Click to check one or more options in the add patient need icon Add Patient Need list: Home Health Agency, Home Infusion, Hospice At Home, Inpatient Hospice, Long Term Acute Care, Medical Equipment, Rehabilitation Hospital, Skilled Nursing Facility.
  2. Clicking right-caret Medical Equipment displays a list of equipment if any has already been added to the case.
  • You can add or remove items on the Medical Equipment list and then search at the same time you look for service providers: see Adding, editing, or removing medical equipment items, next.
  • Any item that is checked checkbox unchecked is included in the search.
  1. Once service providers and medical equipment are all checked off , click search provider.

Adding, editing, or removing medical equipment items from needs

You can add or remove items on the Medical Equipment list and then search at the same time you look for service providers.

  1. Click right-caret Medical Equipment.
  2. Type the name of the medical equipment item needed.
  3. If available, type to add information: type of item, description, and comments. For long comments, click and drag the bottom-right corner of the field to make it easier for you to view.
  4. Click add item. The new item is checked automatically checkbox unchecked to be included in the search.
  • To change the name, type, description, or comments for any item in the list, click edit on the right side of its row, update the text as needed, and click add item.
  • To remove a medical equipment item out of the search, click to uncheck checkbox unchecked its box.
  1. Once service providers and medical equipment are all selected, click search provider.
