The Partners HealthCare Connection program provides access to a personal health coach (via telephone) for patients from MGH and NSMC who have no insurance or who are covered by Medicaid, Free Care, or dual coverage (Medicaid/Medicare). Health coaches can help patients manage their chronic medical conditions, and better adhere to treatment plans. They can also reinforce the importance of medication compliance and preventive care.
Providers answer a few questions and click to electronically submit the referral to the Connection Program managers.
