Adding a Health Form Report

The Health Form Report includes a summary of the patient's active clinical information from data in the LMR. It includes options to suppress specific information, as desired, and to identify specific problems that might affect his or her educational experience. You can also attest to the patient's good health or provide details for how activity should be restricted.

Note: For patient's with Patient Gateway accounts, the LMR sends an email notification to the patient's Patient Gateway account when you sign to save a new Health Form. Additionally, the most recently saved Health Form report displays under the Health Record menu (in Patient Gateway).

1.   If you haven't already, select the patient and go to the Health Form module.

2.   Click Add New.

3.   To suppress specific family history, conditions (problems), procedures, medications, health monitoring, immunization, or physical examination data from report printouts, click to deselect the check box preceding the item. To suppress an entire group of items (such as medications or procedures), click to deselect the header.

Note: For patients between the ages of 13 - 17, the LMR suppresses by default listings of conditions (problems), procedures, and medications.

4.   For the Physical Exam section, use the date field to specify the Exam Date.

5.   Update the lower section of the report by clicking check boxes and typing.

Note: You must specify an exam date and patient activity level. Required fields are indicated by an asterisk (*).

6.   To simultaneously generate a paper copy and save a copy of this report for future reference, click Print and Save. Or to save this report without printing, click Save.

7.   If you want to save and sign your work now, click Sign. The LMR indicates the changes that will be saved. Type your key and click Sign.

Important: Information from LMR reports is confidential material and must be handled and disposed of appropriately.
