LMR Views - LMR Summary

The LMR is now view-only, offering historical data through December 31, 2017. Check Epic for the most up-to-date results.

This view displays a summary of information entered into the LMR for the selected patient.

• To display this view, hold the mouse pointer over the LMR Views menu, and then click the option for LMR Summary.

The view may be blank if either the patient’s providers did not use the LMR or if the providers did not maintain lists within the LMR. It was possible, for example, for LMR providers to use the application to simply dictate notes.

The LMR Summary includes, as available through December 31, 2017:

• Vital Signs: Date, BP, P, RR, Temp, Ht, Wt; if an entry is not available, the system displays ”N/A.”
• Last Appointment date
• Provider name and Practice name
• Active Problem List (Problem, Comments)
• Active Procedures List (Procedure, Result, Comments, Procedure Date, Last Modified)
• Family History (Problem Name, Relative Name, Age of Onset, Living, Comments)
• Active Allergies List (Allergen, Reaction, Comments, Last Modified) Do not assume that a patient has no allergies if none are listed - check the Epic record.
• Active Medications List (Drug name, Stren, Dose, Unit, Route, Freq., Disp, Ref., Start date, L. RX, and Special Instructions)
• Health Monitoring Information (HM Item, Date of Last, Result, Comments)
• Immunizations (Type, Dose 1 through Dose 7 dates)
• To Do List and Reminder List
